DebLin Health Concepts, Inc. is actively recruiting for Case Manager positions in the Houston, TX. The Case Manager is responsible for the implementation of plans at the direction of the Case manager Supervisor for Intensive Case management (ICM) to help each client achieve optimal stability and functioning. He/she familiarizes him/herself with priority client cases, executes tasks in a timely manner, develops resources and establishes a “key contacts” data base for immediate access of resources, familiarizes him/herself with the functions of all DebLin Health Concepts employees and how the CM position can best interact with these positions to serve the needs of the client. CM will implement agency operations, policies and procedure and will ensure that the lines of communication between Administration, the field (therapists, and family members, community providers) and the client remain open.
Minimum Level of Knowledge, Skills and Abilities Required to Perform the Position Responsibilities:
A. Skills in writing, spelling, word processing, data entry and e-mail communication.
B. Ability to calculate numbers.
C. Ability to communicate effectively with DebLin clients via home visits, telephone or in the office.
D. Ability to communicate with and coordinate services between DebLin office support staff, Therapists, Case Managers, Mental Health Specialists, Clients, Caregivers and Managed Care Companies.
E. CPR certified preferred. MA in Social Work or MA Behavioral Sciences
Required experience: Case Management experience minimum of 1 year
